Cleveland County offers the following coverage choices:

HealthChoice Connect

Your online source for claims and benefit information, plus so much more! With HealthChoice Connect you'll be able to access up-to-date healthcare coverage information and resources any time.

HealthChoice Select

A Program, available on all HealthChoice plans, designed to reduce the costs of select services by contracting with select medical facilities to provide these services and bill HealthChoice for a single amount for all costs associated with the service on the date the surgery or procedure is performed.

Under the final rule of the Patient Protection and Affordable Care Act (PPACA), U.S. health insurers and group health plan providers are required to provide a Summary of Benefits and Coverage (SBC) to consumers.

Classen Medical Complex

Cleveland County partners with Classen Medical Complex to provide zero co-pay healthcare to employees and dependents.

Dental Plans

Cleveland County offers the following dental plans:

Vision Plan

Vision Service Plan (VSP)

American Fidelity Flexible Spending Plan

American Fidelity manages this program which allows employees to pay for certain medical expenses using pre-tax dollars.

Disability and Life Insurance

HealthChoice Disability Plan

Provides partial replacement income if you are unable to work due to an illness or injury.

Basic Life Insurance

The County pays the premiums for Basic Life Insurance.  Basic Life Insurance is the first $20,000.00 of life insurance available to employees.  Only employees who are enrolled in one of the health plans offered through Employees Group Insurance Division or in other health coverage that is not an excepted benefit are eligible for life insurance.

Dependent Life Insurance

If you are enrolled in Group Health and Basic Life Insurance, you can elect Dependent Life for your spouse and other eligible dependents.

Family and Medical Leave Act (FMLA)

The FMLA entitles eligible employees of covered employers to take unpaid, job-protected leave for specified family and medical reasons with continuation of group health insurance coverage under the same terms and conditions as if the employee had not taken leave.
